The following was adapted from a "Letter to My Patients" sent October 2007.
I am writing to tell you about the changes in my new medical practice. For the more than 20 years that
I have been a practicing physician, the many changes in our health care system have made it increasingly
difficult for me to maintain the kind of care that I feel you deserve and have come to depend upon. I have
tried to keep you up-to-date with the important preventive measures you need, including advice regarding
diet, exercise, and stress reduction. I also believe it is important to review and coordinate the care provided
by your specialists in order to assist you in understanding their recommendations and in making decisions
regarding your treatment.
In order to maintain this level of care within the current system I have had to make many changes. I
closed my practice to most new patients, and limited the number of patients I saw each day. Even so,
routine screenings for important measures are often squeezed into office visits which have been scheduled
for other things. In addition, I no longer provided convenient services such as in-office blood draws, most
vaccines, and the mailing of prescriptions, forms, reports, and test results because of increasing costs.
Despite my best efforts, I was not able to continue this level of care under my current practice model.
Therefore, on Wednesday, January 2, 2008, I established a membership practice. This new practice is

based on some excellent and innovative ones that select physicians across the country have adopted in

order to deliver the kind of care and service their patients need and deserve. Whether you continue on

with me or if you are a new patient, I promise to deliver the highest level of personal care and medical

                                                                                                                               Sincerely yours,
